Dear Parents,
We are pleased to provide your child with a school-owned instrument for his/her use during the 2023-2024 school year. A maintenance fee of $ 40 is charged annually for the upkeep of our school-owned instruments. At the end of the school year the instrument is to be returned to the school. It can then be reissued for the following year, when a new agreement has been completed. Kindly sign and return this agreement along with $ 40 to the middle school office. Thank you for your participation and support of our music program. Please Note: LAE is not responsible for instruments left in student lockers or unsecured facilities.

Nos complace brindarle a su hijo un instrumento de propiedad de la escuela para que lo use durante el año escolar 2023-2024. Se cobra una tarifa de $40 anualmente por el mantenimiento de nuestros instrumentos de propiedad de la escuela. In consideration of the La Academia De Estrellas releasing the above equipment to my son/daughter, I agree to take full responsibility for the care of the instrument described above, including loss or damage to the instrument beyond normal wear and tear for the 2023 - 2024 school year.
